Skin and Wound Management Module 5: Assessment and Treatment of Pressure Injuries

Licensed nurses are responsible for performing comprehensive assessments upon admission, quarterly, and with significant changes in condition. Assessment for skin risk is an important component of the overall assessment process and should include identifying signs of skin breakdown, wound deterioration, identifying appropriate support surfaces, and describing the types of wound dressings appropriate for the specific wound. This presentation will walk through the assessment process and provide guidance as to preventive measures to initiate if skin integrity is identified as a risk.

Objectives:

  • Discuss the importance of a comprehensive initial assessment
  • Explain physical characteristics of a pressure injury
  • State the signs of wound deterioration
  • Identify considerations when selecting a support surface
  • Describe the types of wound dressings available for treatment of pressure injuries
