Root Cause Analysis

Root cause analysis (RCA) is the process of discovering the root cause of incidents in order to identify appropriate solutions. RCA assumes that it is much more effective to systematically prevent and solve for underlying issues rather than just treating ad hoc symptoms and putting out fires.

As we journey through our day in our centers, many of us are challenged by situations that have arisen, many times resulting in a negative outcome that requires time and energy to address. Some of our biggest challenges of course are related to resident outcomes and falls. Understanding the process of root cause analysis has assisted many centers in identifying risks and successfully reducing those risks to assist in preventing negative outcomes.
This program was developed for you, the expert in root cause analysis or the newbie, just putting your toe in the water. This basic, easy to understand program will walk you through the process, open your eyes to things you might have overlooked and refresh how you view your challenges, armed with new and innovative ways to address them!

Objectives:

  • Identify what root cause analysis is and why it is important in your center
  • Discuss the 5 steps of root cause analysis
  • Identify the 5 “whys” and how important they are to the RCA process

Attendees will be awarded 1.00 ANCC contact hours for completing the course/webinar.

Robin is the president and owner of RLH Consulting which has provided reimbursement, operational, and accounting consulting to long-term care providers since 1998. She is also co-owner of Lake Pointe Rehabilitation and Nursing Center in Conneaut, Ohio, and Quality Life Services, a home healthcare agency in southwestern Ohio. She is a certified public accountant, a state-tested nurse aide, a licensed nursing home administrator, and Resident Assessment Coordinator-Certified and a Master Trainer through AANAC. She is a member of the AANAC Expert Advisory Panel.

Robin has almost 30 years experience in cost reporting, rate setting review and analysis, and reimbursement consulting and representation for skilled nursing facilities and ID/DD providers.

Prior to starting her own businesses, Robin worked for several multi-facility long-term care providers and regional accounting and consulting firms. She held a variety of positions in accounting, reimbursement, finance, and operations.

Robin received a Bachelor of Science degree in Business Administration (BSBA) with a major in Accounting from Bowling Green State University in 1986 and has worked exclusively in long-term care throughout her career. She is a member of the Ohio Health Care Association, the American Health Care Association, the American Health Lawyer’s Association, the American Association of Nurse Assessment Coordinators, the American Institute of Certified Public Accounts, and the Ohio Society of Certified Public Accountants.

Ms. Szumlinski is a nationally-recognized leader in long-term care, with a wealth of experience in virtually every operational and clinical aspect of the industry. She has experience as an Owner/Provider, Administrator, Director of Nursing, Corporate Quality Assurance Coordinator, and independent consultant. In the last 8 years, she has personally conducted over 1,000 on-site risk management visits at facilities across the country, touching every acuity level within the long-term care spectrum. As aA nationally recognized presenter for health care associations on current trends and best practices in the long-term care industry, Ms. Szumlinski is the author of hundreds of best-practice bulletins, in-service modules, and has hosted hundreds of risk management seminars across the country. Respected for success in assisting facilities in achieving and maintaining regulatory compliance, Ms. Szumlinski served two terms as an Associate Board Member for NCAL, is a member of the AHCA Survey/Regulatory Committee and served as a team leader for the AHCA/NCAL Quality Award program for 8 years. Ms. Szumlinski is the Nurse Planner for HealthCap Risk Management’s American Nursing Credentialing Center’s (ANCC) accredited education program.
